Endodontic microsurgery

Endodontic microsurgery treats the tip of a root through the gum. The gum is opened, the end of the root and the infected tissue around it are removed, and the root is sealed from below.

It is for a tooth where a root canal or retreatment cannot reach the problem, or where the crown and post should stay in place. We look first, and many teeth don't need it.

What happens

Surgery is a bigger step, so we explain each part before you decide.

  1. 01

    Step 1: Planning

We examine the tooth, take images, which may include a 3D scan, and explain what the surgery would involve.

  2. 02

    Step 2: Numbing and opening

We numb the area with local anesthetic and move the gum aside to reach the root tip.

  3. 03

Step 3: Treating the root tip

Under the microscope we remove the end of the root and the infected tissue and seal the root from below.

  4. 04

    Step 4: Closing and after-care

A few small stitches close the gum. You get written after-care, a time to come back and a number to call.

Comfort, choices and cost

  • Comfort

We numb the area with local anesthetic. You will feel pressure and may feel the work, and we stop whenever you ask. Soreness and some swelling for a few days is common, and we explain what helps. We don't offer IV or oral sedation.

  • Other choices, and waiting

Other choices are removing the tooth or leaving it alone. Waiting can let an infection spread. We will go through what each choice means for your tooth, and for the cost.

  • Risks and after-care

Surgery has risks: swelling and bruising, bleeding, numbness or tingling near a nerve, an infection, and a tooth that still needs to be removed later. Your endodontist goes through them with you before you decide, and gives written after-care.

  • How it is billed

You get a written estimate before treatment. Our fees page explains insurance, receipts and paying yourself.

Questions about endodontic microsurgery

Is this the same as a root canal?

No. A root canal works from inside the crown. Microsurgery works from the gum, at the root tip.

How long is the recovery?

It varies. We tell you what to expect for your tooth and what to avoid, in writing.

Will the gum look different afterward?

It can, at first. We explain what is normal and what to call us about.
