What do I bring to the first visit?
Your photo ID, your dental plan card and your dentist's X-rays or note, if you have them. The new-patients page has the list.
Where do I put my health history?
On our private patient portal, which the front desk will link you to. Please don't send it by email or in the form.
What if it hurts right now?
Call us. Our urgent-care page lists what to do first and when to go to an emergency room.
